one thing confuses me though, i kinda figured that

but what exactly caused it to happen? the whole part where he gave up souls to stop the huge transmutation circle, or am i missing something else obvious?

Yep, he used most of his stone shielding everyone, and countering the circle. Mix that with his old age, and you have the explanation.
