Those are forgiveable mistakes because they're not caught by spellcheck. I can live with those. It's the ones people let go despite the little red squiggly lines under them that fucking kill me.
On topic:
Long ago, at a law firm my wife worked at, they had an epidemic in the women's bathroom. Someone was shitting all over the stalls on a regular basis. It got so bad that they eventually had to create a sign up sheet to use the bathroom. Grown adults, at a major DC law firm.
Never caught the shitter, the problem just went away.

Whilst it still annoys me, it's vs. its is pretty much the only forgivable one in my book. Affects/effects isn't really difficult at all.

Oh, then/than, that pisses me off big-time for some reason, even though it's hardly a glaring error. Usually I only really notice that one when someone's typed a nice, coherent, grammatically correct passage but managed to fuck up that little detail, which is why it gets to me.
