I've been a member of GameFly for over 2+ years
Some have already stated how to setup your queue.
Start putting games very early, I had GOW III for almost 8+ months in my list
I have the 3 game plan for $30 + tax. I play games on a cycle, while I'm playing 1-2 games the other is being sent back, and new one being received (turnaround is 5-7 days for me)
So I get an endless supply of games
Last year I got 50+ games from GameFly, I spent a total of $400 for the year, so I saved a whopping $2400 if I bought all those 50 $60 games.
My list right now:

I have Bayonetta out right now, so 2 games are freed up. I just finished Darksiders + Dark Void (Both Platinumed). Playing through Bayonetta at the moment, I'll be receiving WKC soon which I'll buy, it will only cost me $40 to keep due to 10% discount + $5 coupon, then BioShock 2 + Dante's Inferno comes out to me, while I'll be sending Bayonetta back, and getting ready for Heavy Rain. As you can see my cycle is set to have games finish and get new ones with-in a week.
(The huge gaps between months = I fill in games that are in high availability + trophy ready)
The game sales + coupon deals help too on GameFly.
I got Borderlands for $30, hell I got Damnation (horrible game, but I'm a trophy whore) for $3 for the PS3. It was on sale for $7.00, I had a $5 coupon, so $2 + tax heh
I just missed out on getting Bionic Commando for PS3 for $6.99 =/
They have GhostBusters for sale @ $11.99
It is worth it to me have GameFly, I have never taken a game yet (telling GF I never got the game, even though you did), and only lost 2 games when I was sent them back (Orange Box + Ferrari Challenge PS3).
I <3 GF when they do there clearance sales, I got Fear 2, Far Cry 2, Shaun White Snowboarding, Borderlands, and Tomb Raider: UnderWorld all together for $50.00, they even gave me a $10 coupon to use, so it wasn't that bad
