I just checked my account history with Squeenix over the last 7 months. I was concerned at first until I added everything up.

$156.63 is what I was billed. The difference is $44.98.
I transferred servers ($25), bought ACP ($10) and got a keyfob ($10).
Additional services were $45.00 approximately. Squeenix just bills out fucked up sometimes. Make sure you check your bank account history before you waste time on hold with them. They've been offering a lot of extra shit lately that loads of people have been buying.
I came back to FFXI after 3 months with WoW, clearing 99% of the content in 3 months with completely ignoring the content of Burning Crusade is boring.
I rejoined late first quarter of June. I went through, checked my billing to make sure my card I wanted to use was in there and reactivated my main's content ID. I also went out and paid for ACP and a server transfer back to the home server I started on.
So far that's $47.94. First thing I do after these transactions is see if my credit card balance updated. With my card, my available credit goes down instantly after a transaction passes and is displayed for me.
No change.
Later that month I bought a security token. Now we enter the new billing cycle for July so I check out my service fees.

I check my credit card and yes my available credit has gone down.
SE does not charge directly for a service that you bought until the next billing cycle. So people that presume that are getting double billed should double check before crying foul and SE should make their billing processes a tad more clear.
It's pretty clear that all billing happens at the start of the month, so adding new IDs and fees to your account don't show up until the next bill.
