The real issue is access. Assume we go to a single-payer system in the future, instead of the multi-payer mix we currently have (Medicare/Medicaid/private insurance, Federal employee plans, VA, uninsured-oh wait they eat those costs in ERs, etc). What do you think will happen to already long wait times to see a general practitioner, who by the way, is much more likely to be a nurse practitioner (NP) or physician assistant (PA) in the future?

The core of the issue is that there is both state and federal regulations. Different laws apply in different states, from what insurance must cover to funding to licensing. What ends up happening is no state has similar insurance, hence pooling is limited on a state level and sometimes on a county level. This causes price fluctuation based on population in any particular region and what particular regulations apply. This is also true for medicare and medicaid. (edit: For medicare gap policies, and medicaid physician availability)

Take bluecross as an example. The company actually split into 50 different companies, one for each state, aka (bluecross of florida, bluecross of conneticut, etc) and each company offers different pricing and in fact, different types of insurance.
