Perhaps both of our viewpoints are just personal opinion. Mr. President is far more respectful, as is anything identifying them as president. They refer to the PM in the UK as Prime Minister Blair or whoever it is now. The founders may not have wanted the president to appear as royalty, but many things have changed from the way the founders wanted.
The President has really become the figurehead of the nation and the way the world sees us is through him. Love him or hate him, the man deserves respect for the job he is doing as it is probably the single hardest job in the nation.

I have no examples off hand. I just know whenever I hear about him it's PM.
Let's put it this way: You call Doctors "Dr. Such-and-such" and teachers "Professor Such-and-such", why not the president?
You don't have to go to school to become President. (hiyoooo)
Because, Mr. Awful Troll, as already stated, President isn't a formal title. They use Mr. to keep it informal.
