Star Ocean 3, Legend of Mana, Radiata Stories, Legend of Dragoon. Not sure if you would consider underrated, but some of my favorites.
When I was a kid I had that stereotypical cheap aunt (everyone has one) that would give the lamest gifts. Socks, hand knitted sweaters that itched to high hell, and knock-off Transformers. One Christmas, she actually got me a genesis game but from a flea market for $2.99 called Shadowrun. I tried it once, picked the guy that looks like Bruce Campbell with a Rambo headband and died in 2 seconds. I tried to trade it in at Funcoland and it wasn't even in their computer. 2 years later, I gave it another go, and got hooked.
Basically the Shadowrun series started on pen/paper D&D style games. You pick from three classes: Samurai, Decker, and Gator Shaman and play through an open-ended story to find out about your brother's death during a mercenary mission known as a shadowrun. Looking back, everything in this game was ahead of its time, Intensive stat building, GTA style missions, party member or gang alliances, weapon customization, computer hacking, and real time combat mixed with random encounters.
I was seriously pissed off when the Shadowrun that was announced for 360 turned out to be nothing but a CS clone. Don't get me wrong, it's a decent shooter, but there's so much more that could have been done to give the title justice.
I'm sure people will mention plenty of ps1/ps2 games so....

i loved the shadowrun games on the SNES and the genesis. the annoying thing about the genesis one was that if you wanted to go decker, upgrading was insanely, insanely expensive. running itself was never very lucrative so you had to use the cheat code (i'd have to do it like 20 straight times to get like halfway upgraded) and sometimes my game would randomly reset and i'd lose my saves.
Secret of Evermore, people playing it expecting a Secret of Mana and then dropping it before it gets good (I've known a good few). But SoE is a gem itself. Try it, quite enjoyable and quite difficult from the get go until the "2nd" world when more helpful/useful weapons/spells become available.

Xenogears and Chrono Trigger. They made a DS remake for the latter but it only adds a couple of HORRIBLE dungeons and a "super boss" fight that adds something to the plot.
Breath of Fire IV is pretty good too.
I just picked up Xenosaga 1 and 2 yesterday on a whim. I hear those were at least fairly epic.
legend of legaia, dual saga was good, but the first was way better
Rogue Galaxy
Can't recall name exactly. lots of spinoffs but the one I remember was for the Sega Genesis. Shinning Force I & II? I think there was a few. you controlled a band of people. mages, archers, warriors. fought across a map grid-like movement. think Final Fantasy Tatics game play.
oh and Panzer Dragoon Saga RPG for the Sega Saturn. I still own a copy of the game and working system too.
Skies of Arcadia was amazing. I'm surprised a lot more people didn't/don't play this game. I got hooked as soon as I started playing it.

Wild Arms series, Alundra (possibly one of the hardest rpgs on PS1), Brave Fencer Musashi (sort of an action RPG)
