Isn't that how Kanye West's Mom died? Basically when you're cut open, you're body reacts to it and can do all kinds of shit to try and counteract it since it can't know the intention of the cut.

Number of possibilities, really. If they're fucking with her ass (harhar), maybe they knicked the artery leading down to the inner thigh. Complications with anasthesia (can't spell this for the life of me) could happen, too. Foul play probably shouldn't be ruled out, either.

Any time you go under general anesthesia there's a chance you'll die. Although it could've been something else unrelated to anesthesia.
Theres also a bundle of nerves in that area of the body - I used to know its medical name, but if this bundle of nerves is cut or damaged a person could suffer serious medical problems such as paralysis.
A person could have an allergic reaction to the drugs they give, there could have been an air bubble that was injected accidentally, they could have cut the major artery (although that runs closer to the femur and hip), the patient could have a previous medical problem that appeared at the wrong time or any number of other things.
There's always some sort of complications, no matter what you do, no major surgery is ever 100% safe.
Losing your life over something like plastic surgery is tragic, why can't people be happy with themselves? It's not worth the possibility of dying for.

Solange Magnano, a mother of twins who won the crown in 1994, died of a pulmonary embolism Sunday after three days in critical condition following a gluteoplasty in Buenos Aires.
