I disagree with you here. Blizzard does a very good job of raid design. They start by making things initially very difficult, then see how the live severs perform then slowly nerf them down to a more reasonable place for most raiders. This is much preferred to say... oh, random nerfs and buffs to AV and PW that cause players to go absolutely crazy. Blizzard starts from one place and then moves back to a comfortable position for most raiders. If it's too easy, do the hard mode.
1) Yes, I do know that they have the PTR to figure these things out. The PTR doesn't tell them everything though, because the average person that plays on the PTR is better geared and higher skilled than the average person on a live server.
2) Yes, I do know that the hard modes for ICC haven't been released, but GC has stated several time that this is their approach to dealing with new content. This was true of Ulduar and even before that. Normal raids are for normal raiders, so they start a little harder and move it back until normal raiders can deal with them. You get better gear from hard modes for a reason.
