People who are addicted to porn use it to the point of it being a destructive force in their lives. They miss work or school, they neglect other social outlets in favor of porn viewing/use, they neglect routine functions (like the kid in the story who couldn't even get it together to stay hydrated through all the fapping), they forgo real sexual contact with others in favor of porn + jerking off, and it becomes the sole most important thing in their entire lives. It's an addiction with the same symptoms as any other addiction. It happens with alcohol, drugs, MMOs, and all sorts of other behaviors that are tools to bring pleasure and/or escape reality.
