Maybe it's just a coincidence or I've skimmed over something, but it seems that everyone who got the title, also got full drops from the kill. Maybe drops are 100% if you get the "true NM"?

Saw this a few pages back and didn't see it answered. I just had mine change color off Krabkatoa, but we didn't receive a title. Had poor drops though.

How hard it Verthandi compared to Orcus for those who have fought both?

Today we beat Orcus with a setup of 12 SMN, 5 BST, and a THF. Overall we only had two people die the entire fight but it was running all over the place because of these new NMs apparent hate issues so that added to the difficulty and damage taken by all of us. If these NMs hate were normal most would be a pushover for SMN burn setup.

Small Fry - tier 1
NM - Tier 2
Shadow HNM - tier 3 in past
Shadow SHNM - tier 4 in past
True HNM - Tier 3 in present

My T3 crystal broke on pop, and I think that's the case whenever you pop a T3?

Most likely not, when you kill the tier 3 and get the tier 4 trigger it will only track down the wyrm.
