I was inspired by all the other email senders, and sent one myself though playonline.com, service and support, contact, feedback and suggestions.
However, lets be honest. You have a better chance at getting a new higher level item (Striped Belt?) than ever getting your Black Belt back. And no, I'm not claiming that the odds for a new higher level belt are particularly good, either.
No, it would be a 10th degree Black Belt and you would need to trade the Black Belt into the npc with more items.
Maybe bercus could email the list in after SE responds to the interview that he sent to them in May 2009. You know, the one where SE occasionally writes back to him to tell him that they are working on it? As recently as December 2009?

For what it's worth, the friend Uryuu mentioned is a LS member of ours. He had his Gjallarhorn and BB dropped in 2006 as well, then character deleted. They screwed up on the restoral process for the character, and so when he got it back, it was over 7 days, and he was SoL for getting items restored. This was back before rollbacks, and when that came out he was not allowed to roll it back that far, so he was again screwed. I tried to assist him in getting either his BB/Ghorn restored, or at least his quest unflagged as completed, and I know he did a whole bunch to try to recover, to the point of being threatened with a banning if he persisted in contacting GMs. Through help of another LS member who has some connections within SE, we found out more concerning the process of item recovery, why they won't restore without evidence it was dropped etc, and what possible options there are to get around it.
By what I have been told, the GMs have to follow policy, up until the level of SLGM (Senior Legendary GM; Sage Sundi is an LGM, and the SLGM is his boss). The SLGM is the only person who seems like he has the clearance of being able to use common sense in making a decision. Throughout the course of a couple of months, and the assistance of this LS member of ours (greatly appreciated by the way, if you are reading this), he managed to get his case heard by the SLGM.
We were able to verify that the quest logs could be used as evidence that Gjallarhorn was completed, despite there not being a visible quest log for it, which was one thing we were really uncertain of. We were at least hoping for Black Belt back, or a quest reset if nothing else, but the potential bonus of the Gjallarhorn was definitely exciting and we had our hopes up after hearing this news (or at least I did).
...and despite all the evidence showing that his character did have these items, the restore was denied by the SLGM, and the quest reset was denied as well. So, best of luck to you. If you manage to get your black belt restored, you will have accomplished the impossible. Your only hope is that they make it a repeatable quest, and honestly I just don't foresee that happening.

All I know is that it was denied, we were never told a reason. At the time of the denial, our LS member couldn't get that information and I never really followed up on it. I'll see if I can find out any more information as to what the exact reason was, but I don't think it matters for us at this point any more. We took it to the absolute pinnacle of the chain of command and we were denied. At least we can have closure on the matter, after almost four years.

For someone who has accidentally lost an item, such as "Black Belt" we should change:
1) The quest “Beat around the Bushin” be changed to Repeatable. Or…
2) That special provisions be made to return items obtained by quests that grant Titles regardless of time lapsed since the item’s loss.
The item is rare/ex and cannot even be sold to NPC's, so verifying that they have the title and DO NOT have the item in inventory should be enough for at least a one time restoral...
