There are probably various statistic suites out there that have auto-coding for qualitative stuff.

Oh yeah! I almost forgot. Google docs allows you to create online surveys that will auto-dump into a spreadsheet. Pretty easy to code from there with find&replace. You can create pretty much any answer format you want and mix them all together too.

For paper handouts and stuff you're stuck with just making it in a word processor and coding yourself.

I'm not sure. You probably want to get that sort of thing from the human subjects committee at your affiliation to make sure it squares with their policies.
