EQ1 was arguably the first mainstream use of them. The concepts obviously go back way further but that's the first place they were widely used in the contexts we know them today, particularly as it relates to the MMO genre.

At it's web peak (don't remember the numbers during the AOL F2P days) the game had a single server, and had about 2k people on at one time. About the same numbers FFXI's Bahamut has running around at any one time (though I've seen Bahamut peak at 4k).

I'd really call Gemstone the first professional MMORPG.
