Hardcores in vanilla were grinding the shit out of what little end-game there was. They still had instances to level up and gear up in, and people may not remember, but XP gains were SLOW in vanilla. It took many months for people to hit max level, and MANY months to get "end-game" achievements like having an epic mount. It used to be just as shocking in WoW to see a max level paladin with an Epic charger than it was shocking to see some JP with a relic weapon or Kclub.
