Are you a cross-server applicant/resided on Asura for less then 6 months?: no but did spend 3 months on Pandemonium farming angel skins for gil

Your previous LS(s)and the reason(s) you left, include LS web URL's and names of your former leaders: Last leader Kazardragon reason for leaving ls has been dying and now down to about 4 people and doesn't seem to be improving any. as for any ls's before than there have been a couple but have been around since 2002 and can't remember them to well but mostly reason for leaving has been due to ls dying

They do seem to have an AH history, but I've honestly never seen or heard of them. Needs to go into detail of who they know, Linkshell names, etc.

Never seen/heard of him or this Kazardragon. Not remembering your own LS' is a red flag.

If all him/her did on pandy was farm 3 months mbee you should find out the original server pre pandy.
