Yes, AFAIK you need to complete My Little Chocobo quest. When I got my Legacy mount on a new character, I couldn't use it until I completed it.

CE Upgrade is available in the account services. Its $19.99 and and they seem to be using a different service to charge accounts. It also has available the subscription options for 30/90/180 day subscriptions. I didn't personally do it yet, so I can tell you the pricing.

You get them for owning 1.0 CE.

Looks like the base level 1 char/server (8 max) is only available monthly @ $12.99 per month. If you're willing to pay for 6 months at a time you can get the same $12.99/month cost for the higher level ("standard") where you get 8 chars/server (40 max). Pretty sure legacy only has the monthly option ($9.99/month for the "standard" plan with 8 char/server, 40 max).
