It used Headbutt 4 times, the first being immediately after a regular attack. The message "Blaize's Copy Image effect wears off" appears when your last shadow is eaten by a 1-hit TP move. The interrupt itself showing in the log 1 second before the second Headbutt actually hits you is just a lag issue. It's that second headbutt that stunned you mid-Ichi and blocked your casting, and the other two killed you.
Lots of times you get a shadow taken before the message "xxxxxx gains the effect of copy images." shows up in your log. It appear to me that that's what happened. Extremely, extremely common. As a tank, you'll see that as often as a mage casting magic (all the time).
Yeah, the log is not an exact representation of in-game events. It never has been.
for easy proof, try casting stun on something high lvl. it's extremely common to see the stun wear off message before the target is stunned message.
Welp, pretty sure I had 1 shadow left before eating the ground, but I also dc'd 3 times while trying to solo it, guess it was random lag.
Logs are never really that great for figuring precise timing, but are you sure you didn't just cancel Ni a little too early? Or just got unlucky with SpellCast/Cancel timing?

So no, nothing out of the ordinary here, just funky chatlog order on the "Soandso casts Utsusemi. Soandso gets Copy Image" lines, which is pretty standard. If you ignore those it looks fine.
Receiving a buff/debuff messages are just always a few seconds late in the log.
Scarscute doesn't use regular attacks, it simply spams the Headbutt TP move in place of regular attacks, sort of like Dark Ixion with his "normal" attacks. I've always found the timing between its "attack rounds" to be extremely erratic, which is what makes him annoying to solo (in addition to the additional effect of Stun and his ability to one-shot you with that other TP move he has whilst he's got you Stun-locked). He hit you twice with Headbutt in quick succession was all.
Canceling utsu2 while casting utsu1 doesn't really make sense on a mob that has 100% hit rate.
He's saying it's easy to time it so your last Ni shadow is taken while casting Ichi, since the mob you're fighting is unlikely to miss. I avoid confidence like that, though =/ it's always my luck I'd get a parry proc or someother and miss Ichi entirely that way.
I thought you could parry TP moves. I remember screenshots of stuff like

gotten plenty of multi-shield skillups when i ate a pecking flurry. each hit in a tp move gets a roll to parry/shield/guard, etc. it's not a big chance as parry is more unreliable than a yugo, but it's a chance.
While mostly true, I believe I've seen him do a melee attack between head butts. It sucks, and completely fucks up shadow rotation. I've duo'ed (Trio'ed... I guess... THF tapped and sat down after putting up TH4) him about 6-7 times.
He also does instant Skull Cracks that occur between his Head butt rotations. They're more heavilly hitting TP moves that don't influence the normal delay on his head butts.
