An altercation that results in a gunfight where the first shot signifies the start. Shots fired used in proper context means someone just got called out and dissed at the same time.

Also when police men are on the radio due to an incident, as soon as a gunshot goes off they radio "shots fired" which escalates police response.

this is what happens when you impose no limits on when it can be used. that is to say, people calling shots fired on shitty disses/call outs that would normally not have even garnered a response.
