well I haven't played it but i'm assuming, because it uses NSMB resources.

It does not have the SMB3 suits, as the person that made it said he just redid the levels and not a programmer, so didn't know how to put them in (As it would require coding to make them function as they should) so as Yuri-G said, uses all NSMB items.

And yes, it works on R4 carts and etc, runs just like NSMB just when you go into levels, they are different.
